Study Abroad Required Forms and Pre-Departure Handbook
The following forms are required for those participating in ¾«Í¯ÓûÅ® study abroad programs unless otherwise noted.
¾«Í¯ÓûÅ® Education Abroad Forms
The need to be reviewed, filled out, signed, and then returned to the Office of Study Abroad and International Partnerships. Completed forms should be emailed to studyabroad@upei.ca.
The forms contained in this .pdf file include:
- Release of Liability, Waiver of Claims, Assumption of Risk and Indemnity Agreement
- Supplemental Travel Waiver
- Risk Management Form
- Emergency Contact Form
- Financial Statement (optional, but recommended)
¾«Í¯ÓûÅ® Emergency Contact Card
Please with your documents in case of emergency.
¾«Í¯ÓûÅ® Letter of Permission (LOP) Form
Note: this form is NOT required for those taking a ¾«Í¯ÓûÅ® faculty-led course.
The must be completed with courses you plan to take while abroad. The completed LOP form ensures you get the courses added to your ¾«Í¯ÓûÅ® transcript once you are done with your studies abroad.
Enter the courses you plan to take abroad and the equivalent ¾«Í¯ÓûÅ® courses (sometimes these will need to be listed as ¾«Í¯ÓûÅ® elective courses - check with the department chairs/deans if you need any assistance). The LOP form gets signed off/approved by the relevant ¾«Í¯ÓûÅ® department chairs/deans for each course. Once you have department chair/dean approval, the form is sent to the ¾«Í¯ÓûÅ® Registrar’s Office for final approval. Upon completion of your study abroad experience, provide the ¾«Í¯ÓûÅ® Registrar's Office with the transcript from your partner school upon completion of your studies abroad.
Yes, you can adjust your LOP form if you have a course you want to add/drop once abroad! Just fill out a new form for the new course, get it signed and submitted to the Registrar’s Office. Make sure to let the Registrar’s Office know if you’ve dropped a course!
